An Italian prosecutor is investigating a series of potential human errors that may have led to the capsizing of the luxury yacht Bayesian off the coast of Sicily in August. According to Constantino, the crew failed to close a door to the jet ski ramp, causing water to flood the yacht and leading to its sinking.
